  • You agree to participate by signing an online informed consent form
  • You are 18 years or older
  • You are in one of these groups: a patient, a caregiver, or a close family member of someone with pulmonary fibrosis (PF) or another lung condition called ILD
  • You live in the United States
  • You have internet access and a working email address
  • You (or the person you’re connected to) do not have certain other lung diseases like sarcoid or cystic fibrosis
