Stroke patient registry for people within 6 months of stroke

Who can take part

  • You are an adult, age 18 or older
  • You have had a stroke (either ischemic, or bleeding/hemorrhagic)
  • This is your first stroke or a repeat (relapse) stroke
  • Your stroke started less than 6 months ago
  • You can sign consent yourself, or your family member can sign for you
  • You are not in a severe acquired brain injury unit and do not have another life-threatening illness
