- You work at a heart transplant and/or mechanical heart pump (VAD) center that is actively performing these services
- Your center must regularly evaluate many patients from different backgrounds and genders for advanced heart failure treatments
- You are an advanced therapy team member at a participating center (such as a coordinator, doctor, pharmacist, nurse, or social worker)
- You may be invited to join the interview part if your name is on the center’s training/meeting attendance list
- Your center must be able to fully take part in the training and evaluation steps
