Who can take part

  • You are 18 years or older.
  • You have a scheduled appointment with a heart doctor at a participating clinic.
  • You have been diagnosed with heart failure in the last 2 years.
  • Your heart failure diagnosis is confirmed by either using a water pill (loop diuretic), a recent heart ultrasound showing low pumping strength, or a note from your heart doctor describing heart failure signs or symptoms.
  • You do NOT have severely low blood pressure, very slow heart rate, advanced kidney disease, certain heart muscle diseases, a heart transplant or a heart pump, severe valve problems, or a congenital heart disease appointment.
