- You are at least 21 years old
- Your primary care provider refers you using an Ask-Advise-Connect referral
- You receive care at one of the participating clinics
- You can use your own smartphone every day (or you can get one through aid help from the study)
- No one in your household is already enrolled in the study
- You do not have an unstable medical or mental health condition
