Who can take part

  • You are 18 years or older.
  • You speak English or Spanish.
  • You have uncontrolled diabetes (A1C of 8% or higher in the past 2 years) or uncontrolled high blood pressure (last reading over 140).
  • You have had at least 2 visits at a participating San Francisco Health Network primary care clinic in the last 2 years.
  • You do not have a terminal illness, severe mental illness, vision or hearing problems that make telehealth difficult, or dementia that keeps you from understanding the study.
