  • You get care at Penn State Health Hershey Medical Center, St. Joseph's Hospital, Hampden Medical Center, Holy Spirit Medical Center, or Lancaster Medical Center
  • You are either hospitalized or seen as an outpatient (ambulatory)
  • You experienced an alert or a diabetes disease-management message during your care
  • The study is not for anyone under 18
