Who can take part
- You have prediabetes, shown by a recent blood test (HbA1c between 5.7–6.4% or fasting glucose 100–125 mg/dL within the last year)
- You are overweight or obese (BMI of 25 or higher)
- You receive primary care at Johns Hopkins Community Physicians in the Frederick area
- You have an active MyChart patient portal account to communicate with your doctor
- You do not have diabetes (HbA1c above 6.4% or diabetes medication)
- You have not previously enrolled in the Diabetes Prevention Program (DPP)
