- You are at least 18 years old.
- You have a suspicious lung mass or a confirmed lung cancer diagnosis (based on your records or clinician judgment).
- You have a history of smoking (current or past).
- Your care is with a participating lung cancer clinician, and you speak English and/or Spanish.
- You have had 10 or fewer visits with your participating clinician so far.
- You can make your own healthcare decisions (no major decision-making concerns noted by the clinician).
