- You had your first stroke and were sent home (or visited the hospital) right after it
- You are 18 years or older and can be scored on the modified Rankin Scale from 0 to 4
- Your stroke type is not subarachnoid hemorrhage
- You and your main caregiver (if enrolling) agree to take part and sign written consent
- You do not currently have recurrent stroke, multi-organ failure, or a terminal stage illness
- You have a mobile phone at home and you are not in another clinical trial
