Who can take part

  • Had a stroke (caused by blocked blood vessel or bleeding) or mini-stroke confirmed by brain scan, with moderate-to-high stroke risk score
  • Going home after hospital discharge (not to another facility)
  • Age 18 or older
  • Mild-to-moderate stroke symptoms (NIHSS score below 15)
  • Good thinking and memory skills (Montreal Cognitive Assessment score above 21)
  • Can manage medications and follow-up care without help from another person
