This study uses a special blood test to look for traces of tumor DNA (ctDNA) and compares it with information from your original tumor tissue or a new biopsy. It’s done in people with locally advanced rectal cancer who are going to receive standard chemotherapy and radiation, aiming to better track the cancer’s behavior.

Who can take part
- You have rectal cancer that is T3, T4, or has lymph-node involvement, but not spread to distant organs
- You are 18 years or older
- You can provide existing tumor tissue for lab testing, or you’re willing to have a baseline research biopsy
- Your overall health is good enough for cancer treatment (ECOG 0–2, meaning you’re not severely limited)
- Your planned treatment includes at least 3 months of FOLFOX, FOLFIRINOX/FOLFOXIRI, or CAPOX, and then long-course chemoradiation to 40–54 Gy
