Who can take part
- You (or your legal guardian) can give written consent to join
- You had confirmed or likely COVID-19, based on test results or doctor findings
- You may have received COVID-19 vaccines, or you may be unvaccinated—either can qualify if COVID was likely/confirmed
- Your medical records include long-COVID-related or vaccine-related post-COVID codes
- No later information should overturn the long-COVID or vaccine-related diagnosis
- Your symptoms must not be better explained by another illness, and you must not be pregnant
