Who can take part
- You live at home (not in a nursing home) and you’re 70 years or older
- You were recently admitted for acute (sudden) care to the hospital unit involved in the study
- Your doctor or care team rates you as frail (clinical frailty score 4 or higher)
- You and a close relative (family member/next of kin) both speak enough Norwegian to answer questionnaires
- You and your close relative both agree (consent) to take part; you must be able to consent yourself
- Your care doctor answers “no” to the “surprise question” (meaning they think your health may be at high risk), and you have not already done ACP for this hospital stay
