This study looks at advance care planning (talking ahead about care goals and preferences) for very frail people who are living at home after a hospital admission. It includes both the patient and a close relative, and may help improve how future care decisions are discussed.

Who can take part

  • You live at home (not in a nursing home) and you’re 70 years or older
  • You were recently admitted for acute (sudden) care to the hospital unit involved in the study
  • Your doctor or care team rates you as frail (clinical frailty score 4 or higher)
  • You and a close relative (family member/next of kin) both speak enough Norwegian to answer questionnaires
  • You and your close relative both agree (consent) to take part; you must be able to consent yourself
  • Your care doctor answers “no” to the “surprise question” (meaning they think your health may be at high risk), and you have not already done ACP for this hospital stay
