Who can take part

  • You are 18 or older and were homeless for a long time before moving into your current home.
  • You now live in a permanent supportive housing program.
  • You speak English.
  • You do not have serious memory or thinking problems that would make it hard to discuss your health care wishes.
  • You can hear and see well enough to take part in a conversation.
