  • You must be at least 12 years old
  • You must have a diagnosis of chronic pancreatitis that doctors consider definite or probable
  • Your first diagnosis must have happened within the last 12 months
  • You cannot be under 12 years old
  • You must not already have a well-established pancreatitis diagnosis for more than 12 months
