Who can take part

  • You are 18 or older
  • You have advanced liver disease, such as cirrhosis or recent severe alcoholic hepatitis
  • During this hospital stay, you had a serious liver-related problem like fluid buildup, confusion, bleeding, or worsening jaundice
  • You are scheduled to be discharged home or to a care facility within 72 hours of signing up
  • You can give informed consent and take part in follow-up by phone or video or in person
  • You have access to a telephone
