Who can take part

  • You are age 70 or older
  • You are considered frail based on a “frailty score” (Groningen Frailty Indicator) higher than 3
  • You have severe low muscle (sarcopenia), based on grip strength, muscle measurements, and physical function tests
  • You were taking 5 or more different medicines before your hospital stay
  • You can follow study instructions (you are “instructible”) and you (or your legal representative) can provide consent
