This trial tests a care approach for Veterans who have long-COVID—aiming to improve symptoms and follow-up care through participating VA clinicians. You may be included if you are a Veteran with long-COVID and can be seen by a participating clinician soon, unless you had severe COVID requiring ICU care.

Who can take part
- You are a Veteran enrolled in VA care (Part 1)
- You have long-COVID (self-report for Part 1, or confirmed using a symptom questionnaire for Part 2)
- For Part 2: you have a scheduled visit with a participating VA clinician within 1–6 months after they are enrolled
- You did not spend time in the ICU for COVID-19 (Part 2 rule)
- If you are a VA primary care clinician: you have not already completed the Concordant Care training (Part 1 and 2 rule)
