- You are 18 years or older
- You have a new (first-time) diagnosis of VT confirmed on an ECG, heart monitor, AED, or device recordings
- The VT episode lasted more than 30 seconds
- You have (or plan to have) an ICD implanted during your current hospital stay
- You have been diagnosed with cardiomyopathy (heart muscle disease, such as scar-related, dilated, hypertrophic, restrictive, or similar causes)
- You have (or will have) follow-up visits at an ICD clinic
