Who can take part
- You are an adult (18 or older) and you sign the consent form
- You need a new dialysis access (hemodialysis) using the GORE ACUSEAL graft
- You are already on hemodialysis or will start within about 30 days after the graft is placed
- You agree to return for follow-up visits as your clinic normally recommends
- You expect to need hemodialysis for the next 12 months
- You do not have certain exclusions, like current body-wide infection or being pregnant/breastfeeding
