Who can take part

  • You were hospitalized with severe COVID-19 between April 8 and July 31, 2020
  • You had a positive RT-PCR COVID test (a lab swab test)
  • Before getting COVID-19, you had a terminal illness and were at high risk of dying soon
  • You were admitted to either an ICU or a palliative care unit
  • Your hospital’s electronic records must have cost information for your stay
