Who can take part

  • You are 18 or older and have cirrhosis (scarring of the liver) diagnosed by tests and your doctor’s assessment
  • You are being considered for a liver transplant (or you are already listed)
  • You score as pre-frail or frail on the liver frailty test (not “robust”)—either LFI or TeLeFI
  • You can read and speak English or French and you have internet access (phone, tablet, or computer)
  • Your MELD-Na score is 26 or lower, and you’re not expected to get a transplant within 12 weeks for a living donor
