- You had a first stroke between 2010 and 2022 (not a mini-stroke).
- It's been at least one year since your stroke.
- You live in the community, not in a nursing home or hospital.
- You are 18 or older.
- If you want the first part: you have Clalit health insurance. For the second part: you were treated at Carmel Medical Center.
- You understand Hebrew, Arabic, or Russian.
