Who can take part
- You must have stage 4 chronic kidney disease (CKD), with an eGFR between 15 and 29 taken twice in the last 90 days.
- You must be 18 years or older.
- You must be able to read and speak English and fill out study questionnaires.
- You must have a caregiver (a relative, friend, or partner who helps with medical decisions and is not paid) who is also willing to join the study.
- You cannot be on dialysis or receiving hospice care.
- You cannot have untreated serious mental illness, moderate to severe dementia, or other active conditions like suicidal thoughts or substance abuse.
