Who can take part
- You are African American or Black
- Your kidney function is very low (eGFR below 29) on two tests at least 3 months apart in the last 2 years, or you have a high risk of kidney failure (KFRE score 10% or more) within 2 years
- You have visited an Emory Healthcare clinic, ER, or been in the hospital in the last 2 months
- You are willing to follow the study plan and be available for the whole study
- You are not currently on dialysis, hospice, or cancer treatment
- You have not had a kidney transplant or been evaluated for one in the last 2 years
