This trial tests a new drug called imetelstat combined with two chemotherapy drugs (fludarabine and cytarabine) for children and teens with acute myeloid leukemia (AML), myelodysplastic syndrome (MDS), or juvenile myelomonocytic leukemia (JMML) that has come back or not responded to treatment. The goal is to find the safest dose of imetelstat and see if it helps control the cancer.

Who can take part

  • You are between 1 and 18 years old.
  • You have been diagnosed with AML, treatment-related AML, MDS, or JMML that has come back (relapsed) or did not go away with standard therapy (refractory).
  • Your current disease has no known cure or effective treatment that improves quality of life.
  • You are recovered from side effects of previous cancer treatments (at least 14 days for most chemo, longer for transplants or radiation).
  • Your kidneys, liver, and heart are working well enough based on blood tests and heart scans.
  • You have never taken the study drug imetelstat before.
