Who can take part
- You are a veteran aged 60 or older
- You have at least three chronic health conditions (like diabetes, heart disease, or arthritis)
- Your physical function is limited (can do 8 or fewer chair stands in 30 seconds)
- You do not have a short life expectancy (less than 12 months)
- You do not have a recent or worsening neurological condition (like a recent stroke or ALS)
- You have no moderate or severe memory problems (score at least 11 on a phone memory test)
- You do not have a medical condition that makes high-intensity rehab unsafe (like unstable angina)
