Peer support for type 2 diabetes in rural Kentucky

  • You are 55 years old or older.
  • You live in a rural area of Kentucky.
  • You have been diagnosed with type 2 diabetes.
  • Your A1c (average blood sugar over 3 months) is 7.5% or higher.
  • You are able to speak English and understand simple instructions.
  • You score well on a short thinking-and-memory test given by the study team.
