  • You must be 18 or older
  • You have a blood clot in your lungs (pulmonary embolism) that was found during this hospital stay, or you have a previous clot that is still there
  • You cannot have a long-term clot condition called CTEPH
  • You must be able to use a smartphone or tablet
  • You cannot be pregnant or breastfeeding
