Who can take part

  • You are 45 years old or older.
  • You have memory problems or trouble thinking clearly.
  • You can use a phone (mobile or landline).
  • You have a friend or family member (18 or older) who speaks English or Spanish and can join you in the study.
  • You do not have a heart condition that prevents exercise, active cancer treatment that disrupts daily life, or a recent head injury with loss of consciousness.
  • You have not had two or more hospital stays in the past 6 months.
