Who can take part

  • You are 18 years or older.
  • You have been diagnosed with heart failure.
  • You have had heart failure symptoms like shortness of breath or swelling (NYHA Class III) and have been in the hospital for heart failure before.
  • Or you have high levels of BNP (a heart stress marker) even if you haven't been in the hospital, or have milder symptoms (NYHA Class II).
  • You are able to keep follow-up visits at the heart failure clinic.
