Who can take part
- You are 18 years or older and live in the community (not in a care facility).
- You can read and speak English.
- You have prediabetes (diagnosed by a doctor, or your A1C blood sugar level is between 5.7% and 6.4%, or a diabetes risk questionnaire showed you are at increased risk).
- If you were previously diagnosed with type 2 diabetes, your blood sugar is now below 6.4% without diabetes medication for at least 3 months.
