Who can take part

  • You are over 18 years old.
  • You are in the hospital because your heart failure symptoms have gotten worse, like shortness of breath, swelling in your legs, or fluid in your lungs.
  • Your doctor has assessed you as frail, meaning you are more vulnerable to health problems (Clinical Frailty Score of 5 or higher).
  • You and a family member or close friend are willing to take part in the study and sign a consent form.
  • You can understand and speak Norwegian.
  • You are not currently living in a nursing home and are expected to live for at least 6 more months.
