  • You are between 40 and 90 years old.
  • You are a patient at a primary care practice signed up for this study.
  • You are already in a care program for heart disease, high risk, or diabetes (type 2).
  • You can speak English or Dutch (if you need to fill out questionnaires).
  • You are not in palliative care or have serious memory problems like dementia.
  • You do not have type 1 diabetes.
