Who can take part
- You are 65 years or older and currently in the hospital.
- You are being treated by a participating doctor at one of five UPMC hospitals in Pittsburgh.
- You are expected to be discharged home (not to a nursing facility).
- At least one of your heart, blood pressure, cholesterol, diabetes, or blood-thinning medications is changed (started, stopped, or dose changed) before you leave the hospital.
- You are able to answer survey questions and give your informed consent.
- You are not enrolled in hospice care.
