Who can take part
- You are between 60 and 74 years old
- You have been diagnosed with high blood pressure (hypertension) for at least 6 months
- You have been diagnosed with type 2 diabetes for at least 6 months
- You received at least one visit or check-up for either condition at the study clinic in the past 6 months
- You can attend follow-up visits for 6 months and provide written permission to join
