This trial provides integrated mental health and substance use assessments for children and teens ages 3 to 18. It's designed for families who've been referred by their primary care doctor and have MassHealth insurance.

Who can take part
- You are between 3 and 17.5 years old at enrollment.
- Your primary care doctor is at one of the three specified Cambridge Health Alliance sites.
- You have been referred to the Family Connections team by your pediatrician.
- You screened positive for mental health or substance use concerns, or your doctor or parent has concerns.
- You are enrolled in MassHealth.
- Your primary care doctor is not at a site other than the three intervention locations.
