- You are 18 years or older.

- You either: (a) do not have diabetes or high/low blood sugar, (b) have diabetes with good kidney function (eGFR 60 or above) and HbA1c between 5.5% and 10%, or (c) have both diabetes and chronic kidney disease with eGFR below 60.
