- Your baby is 16 weeks old (plus or minus 7 days).
- Your baby is generally healthy and has already received the recommended infant vaccines, including one dose of the inactivated polio vaccine (IPV) at 6 weeks.

- Everyone in your household under 10 years old is up to date on their polio vaccines.
- No one in your household has received OPV or an experimental polio vaccine in the past 3 months.
