Who can take part

  • You are between 18 and 74 years old.
  • You have been newly diagnosed with type 2 diabetes.
  • You are a Kaiser Permanente Northern California member in a participating area.
  • If you are 45 or younger, your A1c (average blood sugar) must be above 8%.
  • If you are under 45, your A1c must be above 10%.
  • You are not pregnant and do not have type 1 diabetes.
