- You are a man over 18 years old.
- You have been diagnosed with zero sperm in your semen (azoospermia) from at least one semen test.
- Or you have had a vasectomy and a follow-up test shows no sperm.

- You have not had a previous microsurgery on your testicles to find sperm.
