  • You are 18 years or older.
  • You can use a smartphone or tablet for follow-up appointments.
  • You were just in the hospital for worsening heart failure.
  • Your heart's pumping strength is over 40% (a test done in the past year).
  • Your blood pressure, heart rate, and potassium levels are within safe ranges.
  • You are not already on full doses of certain heart failure medicines.
