  • Your baby is between 0 and 12 months old when diagnosed
  • Your baby has a congenital heart condition that needs surgery or a catheter procedure
  • You are willing to have your baby monitored at home
  • You and your family are able to follow the monitoring plan
  • You have signed a consent form agreeing to participate
