Who can take part

  • You are 65 years or older.
  • You have been diagnosed with heart failure with a low ejection fraction (below 40%).
  • You can speak Mandarin or Taiwanese.
  • Your heart failure is currently stable (not needing emergency care).
  • You do not have other severe health problems like advanced kidney disease, liver failure, or spreading cancer.
  • You do not have severe memory loss, untreated vision/hearing problems, or active mental health conditions that would make it hard to use an app.
