  • You live in the community (not a nursing home or hospital)
  • You have been diagnosed with heart failure for at least 6 months
  • You feel socially frail or pre-frail (e.g., less social contact than you’d like)
  • You own a smartphone and know how to use WeChat
  • You can read and write Chinese
