  • You are the main family caregiver for someone with dementia, providing regular support every week.
  • You live in the same home as the person you care for or nearby and see them often.
  • The person you care for has a recorded diagnosis of dementia from a doctor.
  • The person you care for is not living in a nursing home or long-term care facility.
  • You can read and write.
