  • You are between 10 and 21 years old.
  • You have been diagnosed with type 2 diabetes for at least 3 months.
  • Your recent HbA1c (average blood sugar) is 7% or higher.
  • Your diabetes medications or insulin dose have been stable (no major changes) for the last 2 weeks.
  • You are able and willing to wear a continuous glucose monitor for 6 weeks.
  • You speak English or Spanish.
