- You or your child is Black and not Hispanic
- You have type 1 diabetes and use insulin
- You are currently using (or have used in the last 3 months) an automated insulin delivery system (a continuous glucose monitor and an insulin pump working together)
- You get your diabetes care at UH Cleveland Medical Center
- You have not worked with a community health worker before for help with diabetes technology
- A parent or guardian agrees to take part in the study
