Who can take part
- You have had pain for more than 3 months that is at least a 4 on a 0–10 scale and stops you from doing something you want to do.
- You live in the community (not a nursing home) in Central Maryland.
- You have felt depressed (scored 5 or higher on a depression test) at two separate times over two weeks.
- You are physically frail or starting to become frail (have at least one sign of weakness or low energy).
- You need help with at least one daily task, like bathing, dressing, or grocery shopping.
